ADMISSION CRITERIA

 

An Evolution of Services

 

HILL Program has evolved with time; we began with strict ABI admission criteria for youth, however after observing trends and referrals over the years we have changed to include children, adolescents, and adults with significant emotional and behavioural impairments, which are often interrelated to brain trauma. Some of our clients have also been diagnosed with co-morbid conditions such as Attention Deficit Hyperactivity Disorder, Oppositional Defiant Disorder, Bipolar Disorder, PDD, Attachment Disorders, and Obsessive Compulsive Disorder.  

 

Due to the complex nature of the aforementioned issues, those with neurological, emotional and behavioural impairments require an equally complex and uniquely tailored program to suit their needs. HILL Program offers a wide variety of services that are flexible and geared towards individualized care.

 

PRE-ASSESSMENT INTERVIEW

The pre-assessment interview is a preliminary evaluation conducted by HILL Program to ascertain whether the referred individual is a suitable candidate for placement in the program. The clincial managment team will interview and explore the individual's functional ability in the areas of self-care, productivity and leisure. Individual performance components (i.e. mental, physical, sociocultural and spiritual factors) are considered as they impact on the ability to carry out these essential daily functions. The individual and his/her functional performance is also viewed in the context of his/her current environment. With this information, the Executive Director and the HILL Program team will be able to determine the individual's level of functioning and his/her appropriateness for the program.

If, during the screening process referral to HILL Program is deemed appropriate, the assessment format and location will be determined.

This initial screening process is provided at no cost.

Assessment

The assessment at HILL Program can be performed at a variety of locations and in varying degrees of intensity depending on the predicted level of rehabilitation required upon referral and completion of the initial screening process.

HILL Program follows a functional-integrative approach to occupational performance. The assessment is based on an observation of functional abilities and deficits as they appear in the designated environment. The assessment is performed by the program staff under the direction of the Executive Director, an Occupational Therapist. Consultants in psychology, psychiatry, occupational therapy, physiotherapy, speech-language pathology, social work, special education and vocational therapy may also be utilized to enhance the assessment process based on individual need.

Once the assessment is completed, a report is composed using the functional-integrative approach to occupational performance. Assessment results, an interpretive summary of client performance and a prediction of the discharge outcome are included in the report. Realistic long-term goals, short-term objectives and a comprehensive treatment plan are recommended.

An assessment of longer duration (i.e. to determine appropriateness of residential placement) will also involve a Case Conference with all involved professionals and supports
